Admission Paperwork

Should you wish to place a childcare enquiry to please email info@charliecaterpillars.co.uk with your name and email address.

Your Key Person will complete a Health Care Plan with you at your parent meeting if your child has an allergy, medical condition, special educational need or disability.

We will also complete a Funding Confirmation Of Attendance Form if your child is in receipt of 2,3 or 4 year old funding. Printed copies will be available for when you meet with your Key Person.
